Membership Fee Schedule
All memberships are billed monthly on your sign up date. All clients must agree to the contract. Contracts are a rolling six month timeframe which you may cancel at any time. Contracts canceled early will incur a charge of 50% of the remaining time in your contract (see here for further). Extenuating circumstances may change this additional charge for cancellations.
Please note, we do not take any insurance of any kind but we can still order things through most insurance companies. You must not send a super bill to your insurance as we are not credentialed with your insurance. People who have medicare and medicaid can receive services here but must note, you may be able to find primary care elsewhere where you are not required to pay additional fees.
Additional services called “Wellness Add-ons” are an additional fee.
Any low cost labs or prescriptions purchased through Resolute Integrative Health are an additional fee. Labs and other testing included in your membership price without additional costs include: Urinalysis, finger glucose test, strep throat rapid test, flu rapid test, EKG.

Primary Care Fees

One-time Adult Enrollment Fee $100
Nonrefundable enrollment fee per each adult patient - this will be waved for the first 100 patients that enroll.

One-Time Minor Enrollment Fee $50
Nonrefundable enrollment fee per minor patient who already has an accompanying parent plan - this will be waved for first 100 patients that enroll.

Family Plan (Max $297) Monthly Plan $297
Parents plus minors under the age of 21. Proof of dwelling in same home must be given. For families whose total monthly fees do not meet the $297 max, this number will be prorated for number in family.

Minors Under Age 21 with an Accompanying Parent Monthly Plan $47
We are only accepting minor patients, whose parent already subscribe to a membership. For those 18 and older without a family plan - you may enroll as an adult.

Weight Loss Coaching and Specialty Treatment TBA
Should you request additional weight loss coaching and support that would require more frequent visits, we are developing a plan for this. Weight loss specific visits may incur additional charges to compensate for time. We will provide this information once we have fine tuned our plan. This program will likely include additional specialty medication options.
