Unlimited Medical Visits and Communication:
- Unlimited visits during business hours
- Virtual options when appropriate (California residents must be in Oregon for virtual appointments)
- Phone call, text message, and email correspondence with the provider through our patient portal
- Typical primary care services, women’s and men’s health, family health.
- Additional niche services (TRT, HRT, Medical weight loss, etc) offered for additional fees in some situations
Availability of Nurse Practitioner:
- In office during office hours
- By phone, text, or email after hours (for urgent needs that cannot wait until the next day)
Laboratory Testing:
- Bill your insurance or get wholesale pricing!
Pharmaceuticals:
- Sent like normal to your pharmacy of choice to bill your insurance or….
- Get wholesale pricing on site! Resolute is a licensed outlet pharmacy in Oregon
Procedures Included in Membership
- Electrocardiograms aka EKG/ECG
- Incision and drainage of abscesses if appropriate in office
- Cryotherapy for warts or other appropriate skin lesions
- Rapid strep and flu testing
- Wet mount (checking for yeast or BV)
- Urinalysis testing for infection (culture would be an additional charge)
- Ear wax removal
- Glucose fingerstick
- Simple laceration repair if appropriate in office
- Pap collection (PAP test for pathology is an additional charge)
- Skin biopsy procedure (Biopsy test for pathology is an additional charge)
- Implant removal, IUD removal – not inserted due to costs of device
- Hormone pellets are not offered here
Primary Care Services
- General Primary Care:
- Preventative health – typical primary care for ages 2 & up
- Urgent needs such as viral illness, joint pain, rash, etc
Women’s Health:
- Perimenopause and post menopause HRT support and family planning
Men’s Health
Including hormone optimization if appropriate
Pediatric and Adolescent Health (Ages 2 to 18):
- Includes sports physicals
- Vaccines are not offered due to storage requirements
- Children are only accepted if a parent is also a patient*
Chronic Care Management:
- Diabetes, hypertension, hypothyroidism, obesity, PCOS, etc
Mental Health Care:
- Depression, Anxiety
- For non-complex history
- We do not have counseling on site
Addiction Support:
- For low to moderate risk individuals
Medication-Assisted Weight Loss
For those who need additional support in their weight loss journey, we offer medication-assisted weight loss options which may incur additional charges. Heather will offer both lifestyle and medication options that aid in weight loss by mechanisms such as appetite suppression, increased metabolism, or reduced fat absorption.
Our program includes careful monitoring and regular follow-ups to ensure the safety and effectiveness of the treatment. We combine this approach with personalized coaching and nutritional guidance to maximize your success. Our integrated program ensures that you receive comprehensive support tailored to your specific needs, helping you achieve your weight loss goals in a safe and effective manner.
We do offer several popular options, including in office and at home injections! The pharmacies used are FDA monitored and reputable – an important feature of this treatment plan.
