Monthly Membership Perks
Unlimited Visits and Communication:
- Unlimited visits during business hours.
- Virtual options when appropriate (California residents must be in Oregon for virtual appointments).
- Phone call, text message, and email correspondence with the provider through our patient portal.
Availability of NP:
- In office during office hours.
- By phone, text, or email after hours (for urgent needs that cannot wait until the next day).
- Limited provider vacations planned in advance with coverage available for urgent needs.
Laboratory Testing:
- Full slate of laboratory testing provided at low cost to patients for an additional fee on top of the monthly membership – in house labs ARE NOT billed to insurance.
- Option for lab orders to be sent to local hospital systems for fee-for-service billing through your insurance.
Pharmaceuticals:
- Selection of pharmaceuticals on-site, provided near at-cost to patients WITHOUT billing insurance.
- Prescriptions can also be sent to an outside pharmacy to utilize your current insurance.
Procedures Included in Membership
- Trichloracetic acid for wart therapy
- Electrocardiograms aka EKG/ECG
- Incision and drainage of abscesses if appropriate in office
- Cryotherapy for warts or other appropriate skin lesions
- Rapid strep and flu testing
- Urinalysis testing for infection (culture would be an additional charge)
- Ear wax removal
- Glucose fingerstick
- Simple laceration repair if appropriate in office
- Pap collection (PAP test for pathology is an additional charge)
- Skin biopsy procedure (Biopsy test for pathology is an additional charge)
- Nexplanon implant removal, IUD removal – not inserted due to costs of device
- Hormone pellets are not offered here
Primary Care Services
- General Primary Care:
- Preventative health – typical primary care for ages 5 & up.
- Urgent needs such as viral illness, joint pain, rash, etc.
Women’s Health:
- Peri/menopause HRT support and family planning.
Men’s Health
Including hormone optimization if appropriate.
Pediatric and Adolescent Health (Ages 5 to 18):
- Includes sports physicals.
- Vaccines are not offered due to storage requirements.
- Children are only accepted if a parent is also a patient*
Chronic Care Management:
- Diabetes, hypertension, hypothyroidism, etc.
Mental Health Care:
- Depression, Anxiety,
- For non-complex history.
- We do not have counseling on site.
Addiction Support:
- For low to moderate risk individuals.
Medication-Assisted Weight Loss
For those who need additional support in their weight loss journey, we offer medication-assisted weight loss options which may incur additional charges. Our full program is coming soon. Heather can prescribe FDA-approved medications that aid in weight loss by mechanisms such as appetite suppression, increased metabolism, or reduced fat absorption. We also utilize off label medications that are commonly used to support weight loss efforts. Depending on services, additional costs may be charged and will be disclosed prior to treatment.

We combine this approach with personalized coaching and nutritional guidance to maximize your success.
