Introduction: Welcome to Resolute Integrative Health LLC. These Terms & Services govern your use of our services and outline your rights and responsibilities as a patient. By using our services, you agree to comply with these terms.

Services Provided: Resolute Integrative Health LLC offers a range of healthcare services, including but not limited to primary care, wellness services, and specialized primary care medical treatments including women’s and men’s health. Our services are designed to meet your healthcare needs with personalized attention and care.

Membership and Fees: Our services are provided through a monthly membership program, which includes unrestricted face-to-face visits, communication through our patient portal, and access to a variety of medical procedures and tests. Additional services, such as certain laboratory tests and medications, may incur additional fees. All fees are transparent and will be communicated to you clearly.

Appointment Scheduling and Cancellations: Appointments can be scheduled via text or our patient portal. We request that you provide at least 24 hours’ notice if you need to cancel or reschedule an appointment. Failure to provide adequate notice may result in a cancellation fee if this is a repeated issue.

Patient Responsibilities: As a patient, you agree to provide accurate and complete information about your health and medical history. You are responsible for following the treatment plan recommended by your healthcare provider and notifying us of any changes in your condition.

Privacy and Confidentiality: We are committed to protecting your privacy and ensuring the confidentiality of your medical information. Please refer to our Privacy Policy for detailed information on how we handle your personal data as well as our contract.

Communication: We use secure communication channels to interact with our patients, including phone, email, and our patient portal. By using our services, you consent to receive communications from us through these channels.

Emergency Services: Our services do not include emergency medical care. In case of a medical emergency, please call 911 or go to the nearest emergency room. We provide after-hours support for urgent medical needs that cannot wait until the next business day.

Termination of Services: You may terminate your membership at any time by providing written notice. We also reserve the right to terminate or suspend your access to our services if you violate these terms or engage in inappropriate behavior.

Limitation of Liability: Resolute Integrative Health LLC is not liable for any indirect, incidental, special, or consequential damages arising from your use of our services. Our liability is limited to the amount you have paid for services during the six months preceding the event giving rise to the liability.

Changes to Terms & Services: We may update these Terms & Services from time to time. Any changes will be posted on our website, and you will be notified of significant updates. Continued use of our services constitutes acceptance of the revised terms.

Governing Law: These terms are governed by the laws of Oregon in which Resolute Integrative Health LLC operates. Any disputes arising from these terms will be resolved in the courts of that state.
