“How much does Direct Primary Care cost?” is one of the first questions families ask, and it should be. A membership is only useful if you understand both the monthly price and what that price actually changes about your healthcare experience.
According to MoneyGeek’s 2026 health-insurance rate analysis, a family of four pays about $2,230 per month, or $26,760 per year, for health insurance on average.1 Actual premiums vary by location, age, plan type, and coverage selection, and premiums are only one part of a household’s total healthcare spending.

Our simple monthly retainer membership model means routine primary care does not come with a new bill every time you need care. Individual memberships are currently $110 per month, $130 per month for seniors, or $50 per child. A family membership for a couple and three or more children is $325 per month.

There is also a one-time enrollment fee of $150 per adult and $50 per child.

It is important to think of this membership separately from health insurance. DPC does not replace your health insurance or eliminate your insurance premium.
Instead, your Resolute membership covers the primary-care services included in your plan without billing insurance for those services. Most Resolute patients still have health insurance, which can continue to be used for covered care outside the membership, including additional lab work, imaging, prescriptions, specialist care, and hospital services.
What Does That Look Like Over a Year?
For many people, comparing Direct Primary Care with traditional primary care is not as simple as multiplying a copay by the number of appointments you expect to have each year.
You may only schedule one or two routine primary-care visits in a typical year. But healthcare needs are not always predictable. An illness may lead to an urgent care visit. Resolute+ builds on the Basic Direct Primary Care membership, providing everything included in Basic with an additional level of care for more complex health needs, deeper investigation, and more proactive, data-driven health management.
It is the appropriate tier for higher medical complexity and certain specialty services, such as:
