Same-Day Doctor Appointments: How DPC Gets You Seen Fast

You wake up with a burning sensation every time you use the bathroom. You call your clinic as soon as the phones open, wait on hold, and finally hear the words most patients know by heart: “The next available appointment is in three weeks.”

So you are left with a choice. Wait and hope it goes away, spend part of your day in an urgent care lobby explaining your history to someone who has never met you, or head to the emergency room for something that probably does not belong there.

A same-day doctor appointment should not feel like a lucky break. At Resolute Integrative Health, it is built into how our membership works. This article explains why traditional clinics struggle to see you quickly, how Direct Primary Care (DPC) changes the math, and what fast access looks like in real life for patients in Rogue River, Grants Pass, Medford, and the surrounding Southern Oregon communities.

Why It Is So Hard To Get Same-Day Doctor Appointment

If you feel like appointment waits have gotten longer, you are not imagining it. On a average, it now takes an average of 31 days to schedule a new-patient appointment across 15 large U.S. metro areas, up from 26 days in 2022.[1] For family medicine specifically, the average wait was 23.5 days.[2]

Those figures come from large cities and measure new-patient appointments, so your own experience may be shorter or longer. We have spoken with patients in Southern Oregon that have had to wait even longer than this, in some instances.

The Volume Problem

Traditional, insurance-based clinics are generally paid per visit. To keep the lights on, many providers carry very large patient panels and fill every slot on the schedule. A traditional primary care practice may have a panel of 2,500 patients or more per provider. When the schedule is booked weeks out with routine visits, there is simply no room left for the person who got sick this morning.[3]

Our own FAQ page describes fee-for-service clinics as places that can see thousands of people, with 20 or more patients a day, which makes them very busy and rushed.

The Administrative Bottleneck

Insurance billing adds its own friction. Visits have to be coded, documented for claims, and sometimes approved in advance. Phone lines route through front-desk staff and triage queues. Even a simple medication question can turn into “you will need to make an appointment for that,” because in a per-visit payment system, a conversation that is not billed as a visit is hard to justify.

None of this reflects a lack of caring on the part of traditional clinics. It is the structure. And the structure tends to push people toward waiting, toward walk-in clinics, or toward the ER.

How Direct Primary Care Makes Same-Day Access Possible

Direct Primary Care starts from a different premise. Instead of billing insurance for each visit, you pay a predictable monthly membership directly to the practice. That membership keeps your provider available to you. Resolute Integrative Health does not bill insurance for clinic visits, and our membership includes longer appointments, direct provider access, preventive care, and same-day or next-day scheduling for urgent concerns when available. Here is why that structure frees up time for you.

  • Smaller panels. Because DPC practices are not dependent on visit volume, they typically care for far fewer patients per provider. Recent data puts the average DPC patient panel at 413 patients per provider, leaving more room in the schedule for same-day appointments, urgent needs, and the unexpected.[4]
  • No insurance gatekeeping for visits. Without claims to code and justify, there is no reason to force every question into a billable appointment. Some concerns can be handled with a quick call or message.
  • Direct lines of communication. Our membership includes phone, text, and email correspondence with your provider, plus after-hours phone or text for urgent needs, according to our FAQ page.
  • Self-scheduling. Members can book through our Patient Hub from Elation rather than waiting on a callback.

The result is a practice where “Can I be seen today?” is a reasonable question, not a long shot.

Why Being Seen Quickly by Someone Who Knows You Matters

Speed alone is not the whole story. You can get seen quickly at a walk-in clinic, too. The difference is who you are seen by.

When you walk into a clinic where no one knows you, the visit starts from zero. You repeat your medical history, list your medications from memory, and hope nothing important gets missed. The provider has to make decisions with an incomplete picture.

As our urgent care page puts it, because we already know your health history, we can provide faster, more accurate care. That reduces guesswork and helps you get the right treatment without unnecessary delays. Your provider already knows about your allergies, the antibiotic that upset your stomach last year, your blood pressure trend, and the fact that you have been under extra stress at work.

Timely attention also means less time lost to being sick. Addressing a concern early, rather than three weeks later, can mean fewer days feeling miserable, fewer missed shifts, and fewer disrupted plans for your family.

Real-life scenarios: what same-day access looks like

Abstract benefits are easy to nod along to. Here is how fast access tends to play out for common situations. These are illustrative examples, not medical advice for any specific person.

1) A Sudden Infection

It is Tuesday morning and you have the unmistakable symptoms of a bladder infection. In a traditional setup, you might call, get put on a waitlist, and end up at a walk-in clinic after work.

As a member, you can reach out to your provider that morning. Our clinic is equipped to handle simple urgent needs during regular business hours, including bladder infections, many types of skin infections, colds, sprains, and simple lacerations. In-office testing such as a urinalysis is available, although a culture may carry an additional charge or be sent to a lab that bills your insurance.

2) Experiencing a Worrying Symptom

Maybe it is a rash that keeps spreading, a mole that looks different, or a cough that is lingering longer than you expected. Not an emergency, but not something you want to sit on for three weeks either.

With direct access, you can describe what is happening and get guidance on next steps from someone who knows your history. Our providers generally recommend that any new physical complaint be seen in person for an exam, so your provider will often suggest coming in, and with a smaller panel, coming in soon is realistic. If the symptom points toward something that needs a specialist or imaging, your provider can help coordinate the referral or order.

3) Having a Question About Your Medication 

You started a new prescription and you are not sure whether a side effect is normal. Or you are traveling and realize your refill ran out. These are the kinds of questions that can stall for days in a traditional system.

4) Home With a Sick Child

Kids rarely get sick on a convenient schedule. With a family membership, parents can reach out about a sore throat or fever and ask whether a same-day visit makes sense. In-office rapid strep and flu testing is available which can help answer questions without an extra trip.

Phone, Text, and Virtual Visits: Not Every Concern Needs a Drive

Same-day access does not always mean a same-day drive. For many common concerns, care can be handled remotely through a quick call or message, so you can get what you need without disrupting your day.

  • Phone and text work well for medication questions, follow-ups on a known issue, lab result discussions, and “should I be seen?” decisions.
  • Virtual visits are offered when appropriate, such as follow-ups and ongoing care where an exam is not necessary.
  • In-person visits are recommended for new physical symptoms, because a hands-on exam matters.

One practical note: because Resolute Integrative Health is licensed in Oregon, you need to be physically within Oregon for a virtual visit, as explained on our FAQ page. However, prescriptions can usually be sent to out-of-state pharmacies without issue.

Urgent Care is Included in Your Membership

One of the most common questions we hear is whether a sick visit costs extra.

It does not. 

Urgent care visits are included within your membership.

That matters more than it might seem. When every sick visit comes with a copay or a bill that counts toward your deductible, it is natural to hesitate. Removing the visit fee removes a reason to wait.

A few details to keep in mind:

  • Appointments only. Urgent care hours are by appointment only, and walk-ins are not accepted, so reach out first.
  • Some items cost extra. Certain labs, cultures, medications, and specialized treatments may carry additional costs. The clinic provides estimated prices for in-house labs and prescriptions before collection.
  • Insurance still matters. We recommend keeping a health insurance plan for emergency room visits, specialty care, imaging, and outside testing.

Same-Day Care is NOT Emergency Care

Fast access to your primary care provider is valuable, but it has limits. Resolute Integrative Health is not an emergency department, and some concerns need a higher level of care than a primary care office can safely provide.

If you are experiencing a life-threatening emergency, call 911 or go to the nearest emergency room. Do not wait to message your provider first.

Can I really get a same-day doctor appointment with DPC?

For urgent concerns, same-day or next-day visits are a core part of our membership, when available. Availability depends on scheduling capacity, hours, and whether your concern is appropriate for a primary care setting.

Do I have to pay extra for a sick visit?

No. Urgent care visits are included in membership with no extra visit fees. Some labs, cultures, medications, or treatments may carry separate costs, and you will get estimates before in-house testing or prescriptions. Can I just text my provider?

Do I still need health insurance?

Yes, we strong recommend maintaining health insurance alongside your DPC membership. Your membership covers primary care provided through our clinic, while insurance can help cover larger or unexpected expenses such as emergency care, hospital stays, specialist visits, imaging, outside prescriptions, and testing. Using an HSA for your DPC membership may also be an option, allowing eligible patients to use tax-advantaged HSA funds toward their membership costs.

Can I walk in without an appointment?

No. Urgent care at Resolute is by appointment only. If you are a current member, reach out by phone, text, or the Patient Hub, and we will work to fit you in as soon as possible.

Is same-day access right for you?

Direct Primary Care is not the right fit for everyone, but it is worth a closer look if any of these sound familiar:
